In an effort to provide better care for patients receiving heart valves, On-X Life Technologies, Inc. (OnXLTI), Austin, Texas, USA, would like you to participate in a survey by completing three short forms and viewing an educational video.


Professionally conducted surveys often lead to improved patient care and education. The ultimate goal of this survey is publication.


You must be a heart valve replacement candidate and then receive an implanted valve in order to complete this survey. It does not matter what type or brand of valve you have implanted.


Your participation is valuable. We will compensate each participant a total of $150 for completion of the survey.
